Position Purpose

The Prevention Specialist/Care Navigator will devote 100% of their time to grant funded activities, including HIV/STI counseling/testing/referral, outreach, condom distribution, and providing care navigation to clients who are at risk for acquiring HIV and/or an STI, newly diagnosed with HIV, and previously out of care clients.

Scope of Responsibilities

Vivent Health is one of the nation’s largest providers of HIV prevention, care and treatment services with a budget approaching $150 million and employing over 450 professionals.

The unique, nationally recognized Vivent Health HIV Medical home model of care assures that everyone with HIV has access to medical, dental, mental health and social services and provides the best opportunity for patients to achieve high quality health outcomes. Vivent Health also provides aggressive HIV prevention services to gay men, injection drug users and others at the highest risk for HIV infection.

Vivent Health currently operates in multiple states nationwide. It embraces a Market Culture to increase access to care, enhance quality outcomes, achieve and surpass financial performance expectations and has an aggressive plan to expand into additional states.

 Position Responsibilities: 

  • Achieving all grant deliverables
  • HIV/STI testing and phlebotomy, outreach, condom distribution and special events
  • Providing care navigation to clients who are at risk for acquiring HIV, newly diagnosed with HIV and/or an STI, and clients living with HIV who were previously out of care
  • Completing all required training and attending all assigned meetings
  • Timely and accurate data entry and reporting
  • Assist with the creation of a podcast/YouTube video semi-monthly, and with social media posts promoting the programs and increasing awareness of sexual health issues among the target population
  • Other duties as assigned

Required Qualifications

  • Ability to complete work assignments with minimal supervision, thoroughness, and efficiency
  • Dependability and punctuality in work assignments
  • Valid Missouri Driver’s License
  • Insurable Driving Record
  • Reliable, insured vehicle
  • Pre-employment TB test, flu shot, and COVID-19 vaccination
  • Basic computer competency including Windows, MS Office, and the ability to learn various data management systems
  • Comfort working with clients of various racial, sexual, gender identities
  • Willingness to work evenings and weekends
  • Willingness to drive the Prevention Services Van

Preferred Qualifications:

  • Familiarity with the St. Louis region and its neighborhoods and communities
  • Personal experience consistent with that of the service recipients (especially African American and white MSM)
  • Understanding of and sensitivity to HIV/AIDS, Hepatitis C, and common sexually transmitted infections (STIs)
  • HIV testing certification from Missouri DHSS
  • Certification in phlebotomy
  • Familiarity with and skill using social media
