Position Purpose

The Case Manager provides a wide range of intensive, client-centered services to persons with HIV/AIDS in a variety of settings to expedite access to medical care with the goal of HIV viral suppression, to address medical, psychosocial, and other issues that are barriers to care, and to collaborate with other program and  service providers for the benefit of clients and patients.


Scope of Responsibilities

Vivent Health is one of the nation’s largest providers of HIV prevention, care and treatment services with a budget approaching $130 million and employing over 400 professionals. 

The unique, nationally recognized Vivent Health HIV Medical Home model of care assures that everyone with HIV has access to medical, dental, mental health and social services and provides the best opportunity for patients to achieve high quality health outcomes. Vivent Health also provides aggressive HIV prevention services to gay men, injection drug users and others at the highest risk for HIV infection. 

Vivent Health currently operates in multiple states nationwide. It embraces a Market Culture to increase access to care, enhance quality outcomes and achieve and surpass financial performance expectations and has an aggressive plan to expand into additional states.
  

Position Responsibilities

1.    Provide HIV medical case management services, in compliance with State and agency standards, to persons with HIV infection. This includes:

a.    Provide case management services, according to State Case Management Standards, to persons with HIV infection through assessments/reassessments to determine health and psychosocial needs, service planning; service plan monitoring.

b.    Provide top quality consultation, referral, guidance, and other assistance to HIV-positive individuals, regardless of eligibility for case management, and to the extent required to optimize clients’ success in effectively addressing their needs.

c.    Assess benefits and insurance, and if needed, provide assistance to access payer programs.     

2.    Maintain appropriate client files and timely documentation of services provided.

3.    Contribute as an active member of the department including participating in staff meetings, client staffing, in-service trainings and workshops, department or agency planning activities, assisting in special projects, and providing support, and training to co-workers.

4.    Support the implementation an HIV Medical Home environment in St. Louis, working with a team of physicians, nurses, and other practitioners to optimize access to care by persons with HIV infection and to provide necessary education, support, referral, and guidance so that patients can more readily improve their health status and achieve viral suppression.

5.    Upon the establishment of the Medical Home at the St, Louis location, promote the Medical Home model of care to clients and patients, and provide education on Medical Home services included the EFA Pharmacy.  Assist patients who elect EFA Pharmacy services with enrollment and on-going eligibility.

6.    Work collaboratively with the new EFA Medical Center to ensure that clients obtain appropriate and timely access to all needed care, and to maximize adherence to medical treatment plans.  May participate as a member of the Medical Home Care Team. Work in partnership  with clinic personnel to identify and address issues, to ensure that patients obtain appropriate and timely access to care, and to maximize adherence to and retention in care. Coordinate communication with clinical staff, clinical support staff, and external disease management as appropriate.

7.    Establish a working relationship with appropriate community resources in the area, including those providing HIV services to underserved and target populations.

8.    Work cooperatively with other EFA program staff, inform clients of the services available through EFA and in the community, and link clients to them as appropriate.

9.    Provide education on HIV disease and treatment options to people living with or affected by HIV disease, and to area service providers as appropriate.

10.  Update and maintain a strong working knowledge of HIV/AIDS, treatment options, risk reduction techniques, chronic diseases, mental health issues, insurance and benefits, case management and other related issues through self-study and participation in trainings, workshops, and in-services in consultation with supervisor.

11.  All other duties as assigned by supervising staff.

Required Qualifications

  • Have a minimum of a two to four-year degree in nursing, four-year degree in social work, or a health-related field, such as counseling, sociology, or psychology and experience in locating, coordinating, monitoring services and knowledge of the available health and psycho-social services needed for individuals who are HIV-infected.
  • Ability to establish rapport and maintain effective communication with culturally diverse populations from a wide range of life circumstances and backgrounds.
  • Demonstrated effective written and verbal communication skills, including client-centered communication and assessment skills.
  • Ability to effectively organize work products and files, keep clear records, and manage time well so as to optimize efficiency and productivity.
  • Willingness to travel within the designated territory and willingness to work evenings and weekends as needed.
  • Valid state Driver’s License, Insurable Driving Record, and access to a reliable, insured vehicle.
  • Working knowledge of Microsoft Office and Windows.
  • An Influenza vaccine is required of all new employees at the time of employment, and is required thereafter on an annual basis
  • A tuberculin/TB skin test is required of all new employees at the time of employment, and may be required thereafter on an annual basis, depending on position.


Preferred Qualifications

  • MSW degree and minimum of two years’ experience offering direct client contact in a community based social service or health service agency: experience delivering Case Management services in a medical setting. 
  • Extensive knowledge of HIV disease and related issues, disease progression, and treatment options.
  • Extensive knowledge of medical, social and mental health delivery systems.
  • Working knowledge of Spanish language (bilingual) is a plus.
