Position Purpose

The Clinic Nurse provides nursing care to patients in the Vivent Health Medical Center's Milwaukee location. The Clinic Nurse assists with development and maintenance of clinical protocols, policies and procedures, collects data for the Vivent Health medical Continuous Quality Improvement (CQI) program and provides valuable input for day-to-day operations of the clinics. The Clinic Nurse also participates in research projects requiring a nurse assessment and intervention. 

NOTE: This is a Monday-Friday, full-time position.


Scope of Responsibilities

Vivent Health is one of the nation’s largest providers of HIV prevention, care and treatment services with a budget approaching $130 million and employing over 400 professionals. 

The unique, nationally recognized Vivent Health HIV Medical Home model of care assures that everyone with HIV has access to medical, dental, mental health and social services and provides the best opportunity for patients to achieve high quality health outcomes. Vivent Health also provides aggressive HIV prevention services to gay men, injection drug users and others at the highest risk for HIV infection. 

Vivent Health currently operates in multiple states nationwide. It embraces a Market Culture to increase access to care, enhance quality outcomes and achieve and surpass financial performance expectations and has an aggressive plan to expand into additional states.
  

Position Responsibilities

  1. Provide direct nursing services to patients accessing the Vivent Health Medical Center Medical Clinics, including a comprehensive health assessment, phlebotomy, immunizations and health maintenance screenings. Follow approved nursing protocols and procedures when providing care to patients to meet the nursing needs of patients.
  2. Document all care provided, including but not limited to specialty care, telephone triage calls and medication refills, in the electronic medical chart consistent with standard nursing practice.
  3. Process patient referrals for specialty care and follow-up, as needed, to ensure quality specialty care was provided.
  4. Coordinate communication with medical/social services case management, external case management and other external disease management services as appropriate.
  5. Educate patients on HIV disease, medical care issues and adherence strategies to prescribed medications.
  6. Assist in planning, managing and coordinating patient flow in clinic areas and day-to-day operations of the clinic.
  7. Actively participate in Vivent Health agency meetings including CQI meetings, nursing/medical staff meetings and multidisciplinary meetings.
  8. Develop a strong collaborative relationship with the HIV medical community to enhance patient access to services including specialists.
  9. Comply with the Vivent Health Health Care Corporate Compliance Standards of Conduct and related policies and procedures.
  10. Assist in clinical trials as needed.
  11. Participate in preparing statistical and other reports as needed.
  12. Conduct special projects and perform any other related duties as assigned by the Nurse Manager or Medical Director.

Required Qualifications

  • Wisconsin Unrestricted Licensed Registered Nurse
  • Bachelor of Science in Nursing
  • One year or greater experience in medical/surgical nursing or clinic nursing in a primary or specialty care clinic
  • Certification CPR-BLS
  • Valid State Driver’s License, insurable driving record, and access to a reliable insured vehicle 
  • Ability to type along with a working knowledge of the following software/systems: Word-processing , Spreadsheet, Windows Operating System, Internet & Database
  • Position has a potential for exposure to infectious materials
  • Very effective and practical organizational skills
  • Knowledge of and sensitivity to the HIV/AIDS patient population, and/or an eagerness to learn about this community
  • Ability to work with a diverse patient population
  • An Influenza vaccine is required of all new employees at the time of employment, and is required thereafter on an annual basis
  • A tuberculin/TB skin test is required of all new employees at the time of employment, and may be required thereafter on an annual basis

  • Thorough knowledge of nursing practice standards
  • Ability to travel to other Vivent Health offices
  • Exemplary customer services skills
  • Excellent verbal and written communication skills
  • Ability to be flexible in work hours
  • AIDS Certified Registered Nurse Certification (ACRN)
