Position Purpose  

Provide intensive nurse case management to those living with HIV, promote adherence to medical care, collaborate with other programs within Vivent Health and with local service providers for the benefit of clients. The Nurse Case Manager will coordinate the care of patients across the continuum of healthcare settings including clinic, hospital and the community. This position is supported by the Wisconsin HIV Primary Care Support Network and will be an integral part of its Women's Program.  Medical direction will be provided by the WI HIV Support Network’s Affiliate Medical Director for Services.  Program direction will be provided by the Network’s Clinical Programs Coordinator.

Vivent Health is looking for a full-time Intensive Nurse Case Manager who will provide intensive Nurse Case Management for those with HIV accessing the Vivent Health Medical Center who have complex medical needs by identifying patient/family health needs and developing, implementing and monitoring health care plans to meet patient/family health needs.

The AIDS Resource Center of Wisconsin (ARCW) is one of the nation’s largest providers of HIV prevention, care and treatment services with a budget approaching $120 million and employing nearly 300 professionals.  Our organization also includes both St. Louis Effort for AIDS (EFA) and Denver's Rocky Mountain CARES (RMC).

The unique, nationally recognized ARCW HIV Medical Home model of care assures that everyone with HIV has access to medical, dental, mental health and social services and provides the best opportunity for patients to achieve high quality health outcomes.  ARCW also provides aggressive HIV prevention services to gay men, injection drug users and others at the highest risk for HIV infection.

ARCW operates in the states of WI, CO and MO.  It embraces a Market Culture to increase access to care, enhance quality outcomes and achieve and surpass financial performance expectations and has an aggressive plan to expand into additional states.



Position Responsibilities

  1. Provide intensive Nurse Case Management for those with HIV accessing the Vivent Health Medical Center who have complex medical needs by identifying patient/family health needs and developing, implementing and monitoring health care plans to meet patient/family health needs.
  2. Coordinate the care of patients across the continuum of healthcare settings including clinic, hospital and the community.Collaborate with Vivent Health’s dental, behavioral health, case management and prevention staff and community providers to assess needs, plan and implement care to ensure that patients obtain appropriate and timely access to all needed care and to maximize adherence to treatment plans.
  3. Inform patient of the services available through Vivent Health and in the community and link patients to them as appropriate. Assist with referrals for routine and specialty care.Follow up to ensure that quality specialty care was provided and ensure medical records are received for the patient chart.
  4. Educate patients on HIV disease, other health conditions, medical care issues, lab results and adherence strategies to prescribed medications.Educate family and support persons as well as other team members about the needs of people living with HIV disease.
  5. Assess needs of patients for care at home.Individualize plans for care at home, identify and coordinate necessary and available resources to accomplish home care needs. Act as a resource to home care, community and hospital providers regarding the care of patients with HIV infection.
  6. Plan and implement care in a proactive manner. Provide education to patients on HIV disease, other health conditions, medical care issues, lab results and adherence strategies to prescribed medications.Provide education to family and support persons, as well as to area professionals concerning the needs of people living with HIV disease.
  7. Document all care provided, including but not limited to specialty care, telephone triage calls and medication refills in the electronic medical chart consistent with standard nursing practice.
  8. Maintain accurate, up to date patient records necessary for patient and program monitoring.This includes documentation in Epic, Provide Enterprise and the Primary Care Support Network patient database.Assist with all documentation and information needed for grant reporting.Maintain daily time logs.
  9. Establish a working relationship with appropriate community resources, including those providing medical care, mental health care, AODA treatment and support services to underserved population.
  10. Maintain knowledge and skills relevant to HIV/AIDS with emphasis on issues specific to those living with HIV, through self-study and through participation in trainings, workshops and in services.
  11. Contribute as an active member of the Vivent Health Medical Center, as well as of the Primary Care Support Network, including participating in Quality initiatives, staff meetings, multidisciplinary staffing, in service trainings and workshops, department of agency planning activities, assisting in special projects and providing support, peer mentoring and training to co-workers.
  12. All other duties as assigned.

Required Qualifications

  • Valid Driver's License           
  • Has an Insurable Driving Record           
  • Access to a reliable insured vehicle                
  • Ability to lift up to 20 lbs.       
  • Working knowledge of Microsoft Office and Windows 
  • An Influenza vaccine is required of all new employees at the time of employment, and is required thereafter on an annual basis
  • A tuberculin/TB skin test is required of all new employees at the time of employment, and may be required thereafter on an annual basis, depending on position.


Preferred Qualifications

  • One year or greater experience in medical/surgical nursing, office nursing experience in a primary care or specialty office.
  • WI Licensed Registered Nurse
  • MSN preferable, but BSN or RN with BS or BA in a related field acceptable
  • Thorough knowledge of nursing practice standards
  • Certification CPR-BLS.
  • Willingness to travel within designated territory and willingness to work as needed.
  • Exemplary customer service skills
  • Excellent communication skills
  • Ability to be flexible in hours
  • Effective organizational skills
  • Knowledge and sensitivity to HIV/AIDS patients
  • Able to work with a diverse population
  • AIDS Certified Registered Nurse Certification (ACRN) preferred

Please be in possession of a current WI license as a registered nurse with 1+ year experience in medical/surgical nursing; office nursing experience in a primary care or specialty office; MSN preferred/BSN accepted. A valid driver's license and access to an insured vehicle required.
